Problems regarding Occupational Health and Safety often occur in industrial environments, especially in the printing industry. Implementation of Occupational Health and Safety (K3) is an effort to create a safe, comfortable working atmosphere and achieve the goal of highest productivity. Occupational Health and Safety (K3) is very important to implement because the implementation of Occupational Health and Safety (K3) can prevent and reduce the risk of accidents and illnesses resulting from work. The greater the worker's knowledge of the dangers of work accidents, the smaller the risk of work accidents. One of the methods applied in analyzing potential work accidents is the Event Casual and Fault Analysis (ECFA) and Fault Tree Analysis (FTA) methods. The results of the ECFA analysis are in the form of direct cause (direct cause), root cause (main cause), contribution cause (contributing cause) for each incident or potential work accident, as well as the results of the FTA analysis in the form of unsafe action (unsafe action) and unsafe condition (unsafe conditions) with the results of 12 unsafe actions and 7 unsafe conditions in the discovery of incidents or potential work accidents at PT. Sygma Exa Graphica Bandung City with a percentage of 63.16% and unsafe conditions with a percentage of 36.84% in the findings of potential work accidents.
